what is the transition support programme (tsp)

The Transition Support Programme is a national programme to support disabled children and young people into adulthood. It is part of a wider programme called Aiming High for Disabled Children, which will transform local services in England for all disabled children, young people and their families.

Why
Disabled young people and their families need improved support from local agencies working together. The programme has been developed to make sure support is available to local areasi to work on improving practice in every aspect of transition.

What it is
It is a three year programme with £19m of funding between 2008 and 2011. It is a partnership of the Department for Children, Schools and Families, the Department of Health, and three partners, the national transition support team, National Strategies and the Care Services Improvement Partnership, working with local areas to improve processes to support the experience of transition for disabled young people and their families.
The aim of the programme is to change culture around transition and to raise aspirations of disabled young people, their families and everyone who works with them.
The national transition support team, National Strategies and the Care Services Improvement Partnership together have developed three offers to support local areas in developing further work on transition to improve outcomes for disabled young people.

The offer
All local areas have completed a self-assessment questionnaire and have been matched to a tailored offer of support to help them improve their processes and the experience of disabled young people.

Outcomes
Transition planning and transition services in local areas will better support disabled young people to move towards and on to a new stage of life. Local areas will be supported to meet minimum standards in transition provision. By the end of the programme, many areas will have developed outstanding innovative practice that will lead to improved outcomes for disabled young people. The national transition support team will raise the profile of transition across specialist and universal services to promote disabled young people having the same rights and opportunities as all young people.

The Transition Support Programme is about a better deal for disabled young people and their families. It is about changing culture in local service delivery. And it is about support for every local area to change and improve.

Transition Support Programme
Introduction
The NS TSP Advisory team will work with Regional SEN Hubs to develop and support the implementation in all regions of a self-evaluation framework and toolkit/guidance. The SEF will be designed to integrate all key areas of service input for disabled children and young people and their families into a common outcomes framework, based on person-centred and outcomes-based accountability approaches, to support commissioning by LAs and their Children’s Trust partners to achieve better outcomes for identified individuals.

In the context of Machinery of Government changes (MOG) at locality, sub-regional, and Regional Planning Group level, the toolkit and guidance will provide support for Local Areas to:

  • Establish effective strategic commissioning practice for disabled children and young people (DC&YP) and their families- particularly to enable LAs and partners to promote effective commissioning practice where schools and clusters of schools/settings take more responsibility for universal, targeted and intensive delivery to improve outcomes for disabled children and young people.
  • Improve outcomes and parental/ community confidence in the specificity of need and achievement of targeted outcomes for DC&YP
  • Establish clear accountability across commissioning and delivery partners for their impact on outcomes for individuals and populations of DC&YP across local areas and regions.
