Transition Support Programme in the North West (Update – 2: March 2010)

The Valuing People team are organising 5 key Transition workshops
These events are for people who have a lead within organisations for the development of best practice in transition for young people with learning disabilities. They have been designed to support Learning Disability Partnership Boards to deliver on the Valuing People Now objective, that all young people with learning disabilities will have person-centred transition plans by 2012, and that the plans focus on health, employment, housing and friendships and community inclusion.

The events also address the priorities set by the Transition Support Programme and will be very relevant to people working to improve transition for the wider group of young people with SEN and disabilities.

The dates and focus of the events for your region are:

  • Friday 16th April Person-centred transition planning principles: how to make sure that young people have a strong voice and look forward to full and independent lives **This event is particularly relevant for the Transition Leads**
  • Tuesday 25th May Planning for jobs and careers
  • Friday 11th June Friends and relationships
  • Monday 12th July Planning for good health
  • Thursday 9th September Transition planning, personalisation and support planning
